1. Address for reference : The Works Manager, Government Central Press, Chennai-79.

2. How to apply : Forms are supplied free of cost. They may be obtained from the above address either in person or through postal. Furnishing the correct details called for on the rear of the format helps to publish the change of name in the Tamil Nadu Government Gazette early.

3. How to obtain copies of the Gazette : The person who remits the correct amount as specified in the format will be supplied to his address with five copies of the part of the Gazette wherein the change name has been published. For obtaining further copies, the Assistant Director, Publication Depot, Chennai-2 may be contacted.

Note: Gazettes are retained for a period of one year only for sales. Hence, people are advised to get the required number of copies as soon as they are published.

4. Payment of charges for Notification: Payment towards advertisement charges as specified on the rear side of the application form may be remitted at the cash counter at Government Central Press, Chennai-79 on any working day between 10-00 a.m. to 1-00 p.m. and and 2.00 p.m. to 3.00 p.m. and obtain the receipt for the same. The amount may also be remitted in the form of a crossed demand draft drawn in favour of the Works Manager, Government Centra Press, Chennai-79.

Note: This amount will not be accepted if it is sent by money order.

5. Working days : Monday to Friday

i am sorry to state that the answer above is wrong.
first of all you have to proceed to a notary and get your new name as noted there. then you have to proceed to the district collector. then you have to apply for the state gazetteers through the general administration department.
once your new name is published in the gazette, you have to give an advertisement in atleast three local newspapers for that effect.
after that, you have to proceed for name change in each certificate to the respective board, viz, state board of secondary education for Xth class, Intermediate Board for Plus two certificate, respective university for degree and PG certificates.
if you are married, you have to proceed to the registrar of marriages and district registrar of marriages for vetting your name on the marriage registration certificate along with the copies of the gazette notiication and newspaper adveretisement.
this is a cumbersome procedure, takes money and more so time killing processes.

Procedure for name change in mumbai - India?

You will need to fill up a form which is available free of cost at the Government Press of your city.

You need to fill in the details as required – like your old name, new name, old signature, new signature and address.

No documents are required to be submitted along with the form.

Charges:

Charges for change of name in the Maharashtra Government Gazette:

Rs 120 per insertion (for two copies of the Gazette)
For backward class applicants: Rs 60
Charges for extra copy of the Gazette: Rs 12 per copy
Cash is accepted at the Government Press though you have option of paying through Cheque/DD.

Backward class applicants are required to submit a Xerox copy of caste certificate of old name as issued by the Collector of the District concerned.

Once the form is duly submitted, it normally takes a month for publication of advertisement in the Maharashtra Government Gazette. The Gazette copy is sent to the address given on the form

In case Gazette Notification has already been made and if hyou have received copy of the same, you can submit the same to School. In case you have not received the copy of Gazette Notification, you can obtain the same from the Government Press in your city.

However, if you require an Affidavit, you may contact any Advocate in Esplanade Court, Mumbai or at Bandra Court. They can make the affadavit and get the same signed for you.
