Would it require modification under the hood or would it mount in like it is? Also what are the exact specs of a stock 454 engine. The original production 502 cid 8.2L engine was offered as a fleet option in late 1970s and early 1980s Chevrolet and GMC medium duty trucks. Although it survived as a high performance crate engine, the early 502 cid engine production didn't last long (about 1977-1984 I think) because of the more popular 454 engine. But then production of the 502 was revived in the early 2000s as the GM L18 Vortec 8100 8.1L engine (aka Tonawanda 502 which was built at the GM Tonawanda engine plant), although it is actually a 496 cid engine. But I think production of that engine has ended as well. As for the original factory production of the 8.2L 502 cid engine option, I can recall doing warranty service work on those which was in Chevrolet Kodiak trucks owned by a Pepsi distributor and Chevrolet C60 trucks owned by a U-Haul rental dealer. I don't recall a 502 cid engine option offered in any cars but there may have been a fleet option in some cars as well.

You don't buy that truck for fuel economy, you buy it because you need to haul heavy loads or tow big trailers.

I don't know about the newer trucks but my 1978 K-35 was getting around 9 with a 454/4 speed manual. It gets 11-12 since I upgraded to a newer 5 speed manual. It also has 3.73 gears. I bought mine in high school back in 1984 when gas was only about 60 cents/gallon and still have it, hauling big loads and pulling heavy trailers at least every weekend.

I asked my father-in-law, who owns a 1991 Chevy K3500 crew cab with a 454 and TH-400 (3 speed automatic) and 4.10 gears. He says his gets around 9 or so.
