It depends if your organisation is running these internally, or using a third party service. In the former, it can be assumed they can collect IP data, and track it back to which computer was used to fill it in, or use a network wide monitor and figure it out. Having said that, I believe most managers wouldn’t.About 4–5 years back, I used a free plan from a well known survey company, and they collected IP details in the survey responses, even though we were asking for anonymous answers. That was a little unsettling. I scrubbed this data before having it to my client at the time. The safer method for employees, I believe (however I may be biased), is to use a third party survey tool. The survey company I founded, we had a strange situation occur right after we launched publicly, at the start of last year.We changed the first screen of our surveys to show whether the surveys were being run in Anonymous or Individual (identifiable) mode. One of our customers (who had been one of our testers) was annoyed, because he had told his staff that it was anonymous, when in fact it was set to individual. He threatened to leave if we didn’t remove the warning on the first screen. However, I believe in running a company ethically, and refused to be involved in this deception of his employees. I know, a startup needs customers, however I’d rather have the trust and respect of our customers employees, and so we let him go.The message on the first screen is there to this day; see the image below. The Mode: Individual is linked to a support page about what this means. No, we won't share personally identifiable data with any customer, if the settings are in Anonymous mode. I’d go to court before I handed anything over, and to be honest we scrub most data after the survey closes anyway.Sorry for the rough image, I literally screen capped my phone just now, to illustrate this answer for you.

It is shared and used in aggregate at a team / group / location level.Anonymous - the company won't know who you nor frankly do they care.
