Which is correct, “someone helps me” or “someone help me”?

It depends on the context. If used as an answer to a question such as “How do you keep your house clean?” then “Someone helps me” is correct.If you are asking for help then “Someone [please] help me” is correct.If the latter is the case I strongly recommend that you add please so as not to appear rude.

How do you ask "Can you help me" in French? Is there more than one way?

“Peux-tu/Pouvez-vous m'aider?”“Peux-tu/Pouvez-vous me donner un coup de main?”“Peux-tu/Pouvez-vous me rendre un service?”Variants include replacing “Peux-tu/Pouvez-tu" by the conditional “Pourrais-tu/Pourriez-vous” or even “Est-ce que tu peux/est-ce que vous pouvez m'aider? etc…” although the latter isn't as correct.You can probably find many other synonyms and expressions for “aider/help” all over the French language, but these are the most commonly used formulas.

This is my assignment: Write at least 5 sentences in French about what you like and what interests you about your friends. Each sentence should begin with ce que or ce qui. Make sure you use each one of them at least once.

Can someone help me convert this active voice sentence into a passive voice in french?

"Ce (diplome? prix?) est décerné chaque année depuis 1986" is actually already in the passive voice.
It means "This diploma/prize is awarded every year since 1986"
The active voice would be "On décerne ce diplome/ ce prix chaque année depuis 1986".

"La voix active" is when the subject is doing the action directly himself :
"I love Pauline ":"J'aime Pauline".
"La voix passive" is when the action is being done to you by another party :"I am loved by Pauline ": "Je suis aimé par Pauline".

Edit : Casanova has made a typing error in his answer:
"Passive would be: (Subject) décerne ce prix chaque année depuis 1986". This sentence should start with the word "Active" not "Passive" .
