Reason I say this is RCs are not required to gurantee a return, they may not return anything and you will not be really able to do anything about it. Also if an RC goes bankrupt, or deserts a project after you get conditional Green Card, you are left in mid of nowhere and will have to start all over againYour own project: This is extremely tough, you need to establish a company from scratch and get all required paperwork, work on business plans and everything that a new business needs. Pros is you control your investment and has guaranteed return, profit or loss. Also this step becomes tougher if you are not in country.Job creation: No matter which route you go, after conditional approval you need to show your project has created 10 full time jobs. If your RC shuts down, or your business runs into losses - this can be very challenging.Where as on H1b, all you need to worry about is your job and employer.P.S. It is getting harder and harder to find a residency program that will sponsor H-1Bs these days due to wage issues. So keep that in mind. Even if you find a residency program that will sponsor you on an H, if you end up wanting to do a fellowship elsewhere you may have issues finding another place that will sponsor you on an H.That being said - if your intention is to stay in the US after you complete your medical training, then your best option is the H-1B hands down. H-1B is a straight work visa that easily transitions into a green card. Keep in mind that you can only be on H-1B for 6 years. So if your plans are a 3 year residency, a 2 year fellowship, and another 2 year fellowship - you are going to have issues.The J-1 will be issued by ECFMG (Educational Commission for Foreign Medical Graduates). ECFMG is very strict with their sponsorship. You can be sponsored by ECFMG for up to 7 years which should give you plenty of time to complete your residency and any fellowship(s) that you may want to complete afterwards. You will be subject to the 2-year home residency requirement after you finish your training. This means that you must go back to your home country for at least 2 years after you finish your J program before you can come back to the US on an H, L, or K visa. There are few waivers available for this 2 year requirement that can be very competitive to obtain. The most common one is the Conrad 30 waiver where each state can issue 30 waivers each year to J-1 alien physician trainees to work in undeserved areas in their states for 3 years. During this time you are a conditional H-1B. After you complete the 3 years in the undeserved area then you have 3 more years left where you can work anywhere that will sponsor you for an H-1B. There are very specific rules regarding what jobs are eligible for these waivers and can be very competitive in some states. For instance in Texas the 30 waivers are filled the first day they are available every year but in Alaska they rarely use all 30.
