Does it hurt to get ear wax removed by your doctor?

There are a few ways they remove earwax at your doctor's office. Most doctors I have seen either use tools or irrigate. If he is using tools, then it shouldn't hurt but, the risk is that he can always scratch your earcanal which might hurt. Also sometimes the wax will stick to the ear canal and kind of pull which can also cause a bit of pain.

If the Doctor irrigates the wax out, then it shouldn't hurt but, it can cause you to have vertigo (the sensation of spinning). DO NOT CLOSE YOUR EYES, that makes vertigo worse. Simply pick something to focus on and really focus in on it...the vertigo will leave you in a couple of minutes. Keep in mind that if you use an over the counter cleaning kit. If you get the water too hot or too cold you can give yourself vertigo so, try to make the water as lukewarm as possible and maybe have something you can use to steady yourself or sit down to do it...just in case you start to spin.

The only other way I have heard about is some doctors have a little vacuum system that is used. I don't have much expierence with this method but, I hear from people that it doesn't hurt.

You should let your Doctor know if it does hurt. You might also ask him for alternative methods of cleaning your ears out. Some people just have to go in every 6 months to have their ears cleaned. But, make sure you get them cleaned as impacted earwax can cause a mild hearing loss.

Does reservation (caste based) affect the quality of doctors produced?

There's no objective way of measuring the quality of doctors being produced and whether caste based reservations have any effect if at all on the quality of doctors, so the maximum anybody can do is give personal opinions and anecdotal evidence, so here goes mine.I believe there are many reasons affecting the quality of doctors being produced from colleges, but caste based reservations is not one them. The reason I say this is because although you may get admission to medical colleges based on reservation quotas or management seats, but clearing your college or getting passing marks in your university exams is a different game altogether. In our college, the people who topped in our batch had a mix of people who had come from reservation quota, some from management quota and some general merit students. The medical course is such, your caste or where you come from does not make a difference. For you to pass especially the post graduate courses you will have to work hard, cram stuff learn to treat patients, no quota can help you in this regard.But one area which does get affected is, reservation quota allows some people to enter, who cant handle the pressures of the course that is needed to get a degree. They  keep flunking in subjects, cant clear there practical exams, and in the end either leave the course or sometimes worse end up commiting suicide.Bottom line is the people from caste based quotas, end up passing the course having gone through the same rigours, the long duty hours, getting scolded by your professors for the smallest of mistake, spending sleepless nights studying as the other general merit students. They are as capable as any good doctor. So the belief that caste based reservations affect the quality of doctors is just a myth.

Is it compulsory for a Central Govt. employee to produce a CGHS approved medical certificate for medical leave?

No it's not compulsory.There are a certain set of Rules only according to which medical leave will be sanction to Central Govt. Employees.I am going to explain you in detail.Grant of leave on Medical CertificateRule1: A Government servant (Gazetted or Non-Gazetted) who is a CGHS beneficiary shall be required to produce the Medical/Fitness certificate, ( in the form prescribed in Rule 19 of CCS Leave Rules, 1972, from a CGHS doctor).Rule2: A Government servant ( Gazetted or Non-Gazetted) who is not CGHS beneficiary shall be required to produce the certificate from a Government Hospital or an Authorized Medical Attended.Rule3: A Government servant (Gazetted or Non-Gazetted) who is a CGHS beneficiary, if at the time of illness, is away from CGHS area or proceeds on duty outside the Headquarters shall be required to produce the certificate from an Authorized Medical Attended(AMA), provided that in case if no AMA available within a radius of 8 kms from his residence or place of temporary stay outside his Headquarters and also in the circumstances when he finds it difficult to obtain the certificate from a CGHS or an AMA.Rule4: In the case of hospitalization/indoor treatment permitted in a private hospital recognized under the CGHS/Central Services Rules, 1944, a Government servant( Gazetted or Non-Gazetted, whether a CGHS beneficiary or not) may produce the requisite Medical/Fitness certificate from the Authorized doctor in such a hospital, on account of the particular kind of disease for the treatment of which the concerned hospital has been recognized by the Ministry of Health & Family Welfare.Rule5: The leave sanctioning authority may, at its discretion, secure a second medical opinion by requesting a Government Medical Officer not below the rank of a Civil Surgeon or Staff Surgeon, to have the Government servant concerned medically examined on the earliest possible date.Rule6: The obtaining of a medical certificate does not in itself confer upon the Government servant concerned any right to leave and the Government servant should await the orders of the authority competent to grant leave.Rule7: The certificate shall define as clearly as possible the nature and probable duration of illness.Reference: Book: leave rules made easy by muthuswamy and brinda.Hope you got the answer. The lump in the throat could be an enlarged thyroid gland. The ultrasound will see if that's the case. If a thyroid gland is enlarged, then it's probably not working right. It might be an autoimmune condition, where the immune system attacks the thyroid, so the thyroid has to work much harder to produce the correct amount of hormone. If that is the case, they you can take thyroid hormone orally, no problem.

There are only 9.2 lakhs registered Doctors in india, and the medical council assumed that at any time 80% are working, we get nearly 7.4 lakhs Doctors. (Indian medical register is not a live database).Thus,the Doctor-population ratio comes out to be >1:1600. However, a recent parliamentry committee said that the actual ratio is >1:2000. The accepted global standard is 1:1000. However the ratio gives a false impression.Doctors are not uniformly accessible and the reality is that in govt hospitals , the ratio is close to 1:11000! We have a severe shortage of doctors and that is the reason that Interns,JRs have to work for more than 18 hrs on a regular basis, often stretching to a continuous 36 hrs duty!I think you got the 15 lakhs figure by including AYUSH doctors, but they are not REAL doctors and hence cannot be included in doctor-population ratio.The developed countries usually have a ratio of <1:800 . We are far far behind them,both in terms of number of doctors (relative to the population) as well as quality of care. The quality of care provided at government hospitals are very poor ,and the fact is that we cant even compare ourselves with them!However, the present rate at which we are producing doctors is more than enough, what we need is a good policy to distribute the doctors. There are some states where we have excess doctors and some states where we have severe shortage. Its more of a policy failure than the absolute count of Doctors.
