What causes missed periods other than pregnancy?

I was on birth control starting at age 17. About two years ago I got off the pills, and didn't have a period. After taking several test I realized I was not pregnant and went a total of 8 months without a period. I went to the doctor and she said I had textbook ammennorhea. I was prescribed Provera to take and she said that after a few months of Provera I should be back to a normal cycle and would not need it anymore. I never was able to have a period on my own. After 6 months of needing Provera each month, She prescribed me Ovcon-35 and said that I would need to take that to have a period. That has worked for about a year, but now I am missing periods or having more than one period on the Ovcon. My husband and I would like to start trying to have children. I have talked to my doctor about this and she seems to not be worried at all about me still missing periods and being irregular. What could be the cause of my problems and will it cause problems when I try to get pregnant?

Is she jealous because I had sex with him first?

Was I being derogatory by calling her ma'am?

I work in a customer service call center where 100% of our calls are from customers. We have their account information pulled up on our screens when they call us. We don't know their ages but we do know their names. Let me preface this by saying I live in the south so telling someone ma'am is a sign of respect or at least courtesy.

Ok, this woman called in about her account and she was having some problems understanding the charges on her bill. I said to her, "ma'am, let me see if I can clear this up for you." Even though I had her name right in front of me. This woman ripped me a new one saying "I do not like being called ma'am, sir. I think that is so rude!" At that point I tried to apologize and she wouldn't let it go so she kept calling me sir. I told her that just as good as being called ma'am was negative to her that didn't give her the right to call me sir. Cause that could be taken just in the same tone. She started yelling again and demanded to speak to my supervisor.

I've read several answers here on YA and most people have said that when they are in a store purchasing something they hate it when someone calls them ma'am. Especially when they are under the age of 35 or so. But how are you suppose to know how old someone is over the phone. Personally, I think someone calling me by my name is offensive to me especially when they don't know me in a friendly way. I prefer to be addressed by sir (yes, I'm a guy). That sounds more professional to me.
