How to word nanny duties on a professional resume?

I have been nannying for going on 5 years and I have it listed on my resume. Right now I am responsible for picking the kids up from school, driving to and from any practices, appointments, etc, light housework, maintaining the household responsibilities such as cooking, light cleaning, laundry, grocery shopping. I have my title listed as "household manager" per the mother's request, and I am just trying to find a way to list my duties in a professional manner for an office-type job.

Right now I have it listed as: 1. perform childcare duties for 2 school-aged children, and 2. maintain household duties, schedules, and calendars.

I feel like I am more of the mom's assistant now bc the kids are older, but I don't know how to word that. Whatever she needs me to do she calls or asks and I do it. How do I word this and still have it relevant to nannying without me sounding like I am trying too hard to sound better? I am trying to branch into the professional world because I am going to graduate college in the next month.
