Option #1: Download an mp3 file from limewire or convert any song that you have in a cd, to mp3 format, and then upload it onto ventones and create a ringtone out of it.

Option #2: Go on youtube, find the video of the song you like, and then Ventones will give you an option to convert that song into a ringtone. Just wait a few minutes and the youtube video will download and then you can pick the start and stop points to convert it into a ringtone.

For verizon. When you press the send to phone button after you make your ringtone, wait like 10 mins. If you don't get the ringtone, just download the ringtone to computer, and then send the ringtone to your email address which is number@vzwpix.com

You’re looking for a Audio to MIDI converter (see link below). You can get hardware or software for this, but software is probably your easiest option. Then you can take those notes and convert them to sheet music using many different audio programs like Midi Sheet Music which is free.
