Both parents must come in person with the child and should bring the following documents (all originals):

1. Child’s Brazilian birth certificate;
2. Parents’ marriage certificate – if applicable;
3. Divorce final decree of parent’s previous marriages – if applicable;
4. Satisfactory evidence that the child is the natural child of the mother whose name appears on the Birth Certificate. This may be in the form of medical reports covering pre-natal care, a doctor's signed statement attesting to pre-natal treatment, mother’s pre-natal exams or other evidence as may be required by the Consular Officer;
5. Evidence of the parent's American citizenship. This may be in the form of a valid American passport, or the original of a birth certificate showing birth in the U.S., or a U.S. Certificate of Naturalization. If one parent is not a U.S. citizen he/she must present his/her passport and a Brazilian ID Card if he/she is a resident of Brazil;
6. Child’s Brazilian passport (if already issued);
7. Parents’ passports;
8. Hospital records where the child was born;
9. Family photos showing the children growing;
10. If one or both parents have been previously married, the original of the document terminating such marriage(s), i.e. Divorce Decree, Death Certificate, etc.

At the time of your appointment both parents and the child are required to appear in person at the Consular Section.

There is two way to change his name. in Both way you have to go to City Civic center.If his age is less than 18 just make affidavit for change the name on stamp paper as his parents and submit it along supporting documents to Birth- death record department in municipality office or nearest civic center of area. It will take 15–30 days to change in official records.If His age is more than 18 he should self apply for change the name in municipality as per above option 1.Note : If you want to change the name in Ration card, They advised you to declare it in Gazette than apply it again with copy of it. Wanted to add that the vitamin mentioned above is actually gven in a shot. It is vitamin K and it helps with clotting. The shot and the ointment are given at the same time so that is why the mom was confused.

I pierced my baby's ears the second day she was born in the hospital. But there are many hospitals who don't do it...

My baby is 11 months old, and her ears are perfectly fine.

My mother also pierced my ears at the hospital too, and all the baby girls that I know also did it in the hospital.

Call the hospital in advance, there are medical earings that you have to put for the first year to prevent ear infections, and you have to turn it everyday after shower for the first 3 months. "Gold earings and other kind of jewelry is not recommended"
