A survey is essentially a type of interview, so you can cite it in a similar manner (see "Interview" citation type in NoodleBib). If the survey had a name, you can provide that as well.

Last name, First name. "Name of survey." Survey. DD Month YYYY.

As an example:

If the survey or poll was conducted online, provide the URL and date of access as well, as you would any other online citation.

book:

Author's last name, First name. Title of book (in italics). City where published: Publishing company, Date.

Bibliographies need to be in alphabetical order, double spaced (within and between entries).

Oh god, annotated bibliographies. They're a hassle to write (at least for me), but they are useful and have a good reason for existing.An annotated bibliography's purpose is to inform the reader about the content, quality, veracity, etc. of an article. First you have the citations, those are fairly easy. I’m going to assume you want to know about the annotation part, yeah? So first you'll need the citation, then a brief summary of your source, then your evaluation of the source. Be clear, direct, and concise in your summary.Now, here are some good things to put in your annotations:Past achievements or qualifications of the author.
