That I have requested the [Name of Bank] to register my new Specimen Signature on record, which is affixed hereto and shall hereafter be my signature and also to cancel/delete the signature provided by me at the time of opening the aforesaid account. 5. That I hereby affirm that I shall not hold the Bank in anyway responsible for any consequences that might occur directly or indirectly on account of the Bank acting on my request herein stated by replacing the signature(s) in the Bank's records and for the consequences acts /deeds by the Bank and I affirm that I shall indemnify the Bank in caseĀ  of any resultant loss.That this affidavit is drawn to be submitted before the [Name of Bank and Branch] for my signature verification.I hereby state that whatever is stated herein above are true and correct. If I use someone else's social security number to work, will they find out ?

Social Security Administration has a special website available to employers to verify social security numbers of all employment applicants. It is keyed to the name, social security number, and the birthdate. If those three don't match the system rejects and the employer is supposed to not hire the person or get more information.

The STORE will find out you lied on your application right away. Or at least within the year if they don't use the SSA system. The OTHER PERSON may not know for years that you have been helping him/her with his/her retirement benefits.

Use your own SSN and avoid a lot of trouble for yourself. If you are trying to hide for some reason maybe now is the time to stand up and take care of the problem.
