Do i fill out form DL 44 for the dmv?

Yes, regardless of age the application must be filled out.

Driver License or Identification Card Application DL 44

The application to obtain a driver license or identification card is called “Driver License or Identification Card Application” (DL 44). The application to obtain a commercial driver license is called “Commercial Driver License Application” (DL 44C). Use these forms when you apply for, renew, replace (duplicate for a lost or stolen driver license or identification card) your driver license or identification card. You will also use a DL 44 or DL 44C to change the current class of your driver license.

The department does not provide the DL 44 or DL 44C online because original signatures are required on the form, and each DL 44 or DL 44C contains a unique barcode which must be scanned or keyed by the DMV employee.

To obtain a DL 44 or DL 44C form, call DMV’s Automated Telephone Service at 1-800-777-0133 (available 24 hours a day, 7 days per week) to have a form mailed to you or visit your local DMV office to pick one up.

Does filling out these forms mean that I am hired?

They should already be paying you. But I believe they are skating in a twilight zone of keeping you on but not calling you an employee. The confidentiality form and invention rights agreement I would argue put them over the line and I’d push them or even go to the state labor board. Possibly spend some dollars with a labor attorney to answer this question.Think of what were to happen if you were to- while waiting for them- build a website that started to make a lot of money. They’d be on you for their perceived ownership in a heartbeat and they’d point to that agreement.I personally won’t fill out an I-9, W2, etc until and unless they have hired me and I am expecting a paycheck at that point. Otherwise its not only a waste of time its also an exposure of some fairly private information.Just because they are delayed getting you a badge doesn’t mean they shouldn’t be paying you. This happens all over the place where someone gets hired but getting them fully onboarded takes time. In those cases the usual procedure is for the employee to try to make themselves available but often they are sitting on their hands waiting. But still being paid.Look through the paperwork and see where it says this is contingent and what the language says.I’d go get another job. I won’t work for people like that.

How to file my tax information for a job?

I got hired for a job at Wendy's last week, but I need to file tax information before I can start. My employer said she has sent me an email multiple times and I still can't find the email. I tried entering two different emails on the employment website but no luck. I can't figure out how to file my tax information for this job! I have called them multiple times so I don't want to keep bothering them, and I did tell them that I really am interested in the job and that I hate to keep bothering them, but I just can't figure out what to do about my tax information! Please help!!!

Do people stationed in the military need to fill out a form about anyone they have contact with? If so, what would it entail/be called?

Nope. The military is not like prison. You can have your own friends and family. There only time it could matter if a military person has a security clearance when they do a background check. Even that is free. You don't have to pay for any administrative actions. There is no charge to process paperwork. If that was the case the government would never get anything done. You are speaking with a liar and criminal. This person is NOT in the military and is trying to scam you. Stop all communication and book him.

How is it that when you fill out a form, "Asian" is somehow listed as one race?

What's the purpose of the form? You need to know that before you decide how many categories it should have and whether identifying the specific Asian category would make the results more meaningful. In many cases, the only purpose is to gather data about the diversity of a workforce, or a student body. In that case, a few categories are thought to be sufficient. There are many reasons to have detailed statistics about the population of an area. I can't speak to other countries, but the U. S. census form is extremely detailed. Anyone who wants very specific information about the population of an area can get it. It's true that the “Asian or Pacific Islander” category is grouped in a lot of public reporting, and I suspect that's because in many communities the percentages would be too small if it was broken down further.Source: The 2010 Census Questionnaire

Does your doctor's office charge to fill out forms?

I had an injury occur at my home and when I returned to work, I was requested to fill out FMLA forms to cover my absence. I just called over to my doctor's office and they told me that they charge $5 per page.

Can they do that? Does anyone else's doctor's office charge to fill out papers?

I know my OB/GYN [I had FMLA forms for my maternity leave 2 years ago] and my children's pediatrician doesn't charge to fill out forms, just my GP.
