How do you answer: "Where were you born?"?

Depends on who I am talking to and where I am...

If I am in another country, I say, "The USA."

If I am states away from home in the USA and talking to another person from the USA, I say the state I was born in.

If I am in the state I was born in, I say the city I was born in. The reason is because the person would know where that city is whereas if I was states away, they might not.

As far as your actual question in reference to a hospital, etc... most applications ask for the county not the city for this reason. Usually they are the same. If they aren't, you need to put the county/city where your birth certificate is stored in the registrar of deeds database. That is why the question is ask... to verify your records.

I was born in Cleveland Ohio and my parents were both born in Belleville Illinois. I was there because my pregnant mama wasn't stupid enough to stay home but went with my very attractive dad on his job to Cleveland.

For my kids, it was the city they were born in. (Like a lot of fathers who were deployed a lot, I have trouble remembering who was born in the Base Hospital, and who in town; don’t mention this to my wife.) God alone knows what they now call the old Soldiers’ and Sailors’ Relief and Benefits Act, but it and its successors allowed service members to maintain their permanent place of residence at the place where they first entered service, but as far as I remember, my sons were citizens of Massachusetts, and my daughter may have been an Ohio citizen, born at Lockbourne(?). I’d appreciate comments from someone who understands this better. I do remember that the Columbus Registrar’s office insisted that I state where I intended to be buried, before they’d put me on the rolls as an Ohio voter from Hamilton Township, because up to Date of Separation, I was still a citizen of Massachusetts.

You do not need to know the city. In most states, the marriage license application only requires you to show the state, or for foreign-born people, the country of their birth.

Does your mother have a birth certificate? That would show it, of course. For your father, "unknown" is the only answer you can give.

There are online resources, including a tremendous database run by the Mormon Church, which anyone can access. But it is a ton of work to do the research, in my experience, and I do not think the license bureau will insist that you have it, especially if you know the state or foreign country.
