The application for re-issue of passport with the changed name or surname needs to be supported with the requisite documentary proof and evidence. How do people apply for a passport if they have a different name on their birth certificate than the name on their other documents?
Thanks for the answers. I have already received my passport now, this is how it went..I made a call to online support of Passport Seva. They told me it's up to the Passport officer only which documents are needed although they told me if I can apply to change the name on my birth certificate. So I went to the Municipal birth & date registrar office but they told me as per the new circular, the name can be changed only if it's appearing as a Freudian slip. So again I called the Passport online support & then they told me to download Annexure D available on Passport Seva website which states the name change issue & make an affidavit or notary from notary officer and issue an advertisement in 2 news papers, one that is in your local area and one that is available nation wide.So I took these documents with me when I had my appointment for Passport & they accepted my application without any problem.The only thing I can conclude is that it's really up to the Passport officer, for some it worked with just a simple affidavit and for many it got rejected as I saw that in the Passport office. So it's better to be proactive.
