Creatinine is supposed to be in the urine, it's a waste product. If there's more of it than normal, it probably just means the urine was a bit more concentrated than normal. Remember the lab test only measures the CONCENTRATION of it, not the total amount that is there. Maybe he hadn't drunk as much as usual. Creatinine is a by-product of creatine and creatine phosphate metabolism in muscles, high creatinine levels is associated with big muscle mass. Body builders may have higher creatinine levels than let say a marathon runner. Elevated creatinine levels in blood, signifies impaired kidney function or kidney disease. As the kidneys become impaired for any reason, the creatinine level in the blood will rise due to poor clearance of creatinine by the kidneys. Abnormally high levels of creatinine thus warn of possible malfunction or failure of the kidneys ( CKD, ESRD,Renal failure). Your creatinine level may temporarily increase if you're dehydrated, have a low blood volume, eat a large amount of meat or take certain medications may cause acute kidney injury which is detected by looking at BUN/SCr ratio.
