It might be used in a letter, for instance a reply to an enquiry which been delayed; but most often in a spoken context, to apologise to someone who has been kept waiting. A variation could be “Thank you for being so patient [while I found your file …].I'm afraid I don't know where it originated, it is not a particularly unusual expression. Unfortunately I can see where it is heading: into the dustbin of meaninglessness with all the other insincere customer service formulas like “have a nice day” and “your call is important to us”.
