Why should you maintain a record of sources you have used and how to do so?

Basically its so that you dont get done for plagiarism...
So they know its your work and you haven't just copied big chunks from someone else.

If you mean, "Why were fossils put in the ground?" then you're begging the question by assuming the fossils were put there with a purpose. They just happen to exist, and they just happen to have been found by a species inquisitive enough to find them. Fossils have been around longer than humans.

However, if you mean "What do humans use it for?", that's a different question. Perhaps the biggest use is that we can date the fossils and construct an idea of what life was like at the time the organism was alive. We probably wouldn't know much about the history of life if it weren't for fossils.

No question, the electronic health record (EHR) should be the source of patient data, but that is not the case. The problem lies in the definition of “the electronic health record”.
