What Should I Used To Keep A Record Of Employee Certifications

Where do I get CERTIFIED medical records?

You know you aren't supposed to carry your Social Security card, right? It's supposed to stay in a lockbox with your birth certificate and passport.

http://www.mbc.ca.gov/forms/enf-22.pdf is a copy of the CA State medical record certification. Basically, the medical office prints out a copy and certifies that what's there is true. Where I see a problem is that many offices require an ID to get your medical records. You'll have to call and see where that puts you. While I don't know that it's a legal requirement, getting an attorney involved is going to be more difficult than just showing the Social Security office your medical insurance card or your school ID.

You should also look into getting your passport and keeping it safe somewhere. They are a bit expensive, but they basically serve as absolute ID and can be used to verify citizenship and identity for most federal forms (like when you get a new job), for travel (even domestically), and for just about anything else. In other parts of the world, it's standard to use them as ID yet in the US only about 1/3 of citizens have one.

Firing employee for calling in sick too many times?

I inherited this problem a month ago. The former supervisor wrote her up and gave her a warning. He said, "I sick of dealing with this sh@t" and then he quit. I'm her manager now, and in 5 weeks, she called in sick 6 times. I spoke to her already that it is not acceptable. Her respond was I'm sick and there is nothing I can do.

Is it legal for your employer to ask for a COPY of your social security card?

From the official instructions for form I-9, verifying new employees eligibility for work:

*****

Employers must record:

Section 1, Employee: This part of the form must be completed at the time of hire, which is the actual beginning of employment. Providing the Social Security number is voluntary, except for employees hired by employers participating in the USCIS Electronic Employment Eligibility Verification Program (E-Verify). The employer is responsible for ensuring that Section 1 is timely and properly completed.

1. Document title;
2. Issuing authority;
3. Document number;
4. Expiration date, if any; and
5. The date employment begins.

Employers must sign and date the certification. Employees must present original documents. Employers may, but are not required to, photocopy the document(s) presented. These photocopies may only be used for the verification process and must be retained with the Form I-9.

So, your boss may ask to see your original social security card and can make a copy for your employment file if they want to.

Potential Daycare asks for SS cards and birth certificate?

Thanks for the comments! Bells and whistles went off when the director asked for it as part of the required paper work while touring the facility but I wanted to make certain.
I will not submit that information. Even more, I will look elsewhere for an extended hour day care facility.
