I know you don't want to hear this, but you really have to start by addressing your issues with the teacher himself.
If you don't then you will be directed to by the principal. Its uncomfortable, but its how conflicts are best resolved. To refuse to do so makes it look like you are on a vendetta instead of having real issues you want resolved.
Nothing you mentioned by the way, would get a teacher fired. So be aware of that and be ready to say what it is you would like to see happen...
1. The teacher shows up on time.
2, The teacher use respectful language in class.
3. The teacher make clear the requirements and methods used to pick award recipients.
4. You would like an administrator to monitor with surprise visits regularly.

So i have a Math Teacher Ms.Pottash, i Want to file a complaint against her. She Discriminates me, shes unfair( mon. she sent me to the dean 4 dress code violation, stripped sweater, and other students had to go as well. she crossed the names out and put me on instead. all the other students were wearing colored under shirts, jeans and not on a fri. and had logos the size of a cooking pan this is violating rules)

She pins me down in front of the whole class for failing a test and discourages me, saying im lazy and that i cant seem to do any better

First of all, I find it hard to believe that the principle would not do anything about it. I would ask the other parents in of the children in your child's class if they have heard similar complaints. If they have, get a group of parents to go meet with the principle about it, or you could go directly to the teacher and ask if this is happening. If the parents' children have not heard anything from there children, maybe your child was exaggerating or just doesn't like the teacher. Ask your child how often the teacher yells, etc. (although most teachers do yell quite frequently in a rowdy class and that is probably not much to worry about) and if it is all the time, throughout the day or week or if the teacher is physically aggressive and the principal refuses, I would try to contact the superintendent of Catholic schools. I am not saying this should go unnoticed, and I do not know the whole story, but from your statement above, it does not sound like something you try to take farther than the Archdiocese. I'd say the talking to the teacher and other parents would be your best bet, then the superintendent.
