This requirement leads to a template very similar to the one above except that it includes all the header images as well as any appropriate text.Here’s a look at the ‘only-lives-in-Word’ letterhead that we use. Top to bottom and left to right what you see on the first page is:A left justified white-on-back header that reads ‘2SS.Com.’ (It used word’s text shading feature.)A descriptive phrase positioned under #1 above that reads “a comprehensive computer services firm” .A Word field that automatically insert’s today’s date.Our right-justified logo followed by all our contact info. What is the standard size of logo in a letter head?

There isn't one answer to any of that. You can decide for yourself where to put those things because nobody sets guidelines for those things.
