How do I get a birth certificate with my new legal name?

Hi there, I know this is an old question but for those people out there looking for answers as I was too, here it is. After changing my daughter's legal name which was an annoying process and very costly (although MA have waivers for those who qualify), after several process I got the approval by the court to change my daughter's name. After I went on in a search on how to make the change on the birth certificate, went to city hall of both town where I live and town where she was born, went back to the court, went to the "Massachusetts Vital Records Dept., and all of them gave me the same answer. The law varies in different states but in our lovely MA, IT IS NOT allow to change the name on the birth certificate, no matter what legal document states that you have a different name now. I was told by the MVR Dept. that until MA change the law, there’s nothing they can do to change a birth certificate name. Which if you ask me, it is absolutely ridiculous and absurd that after the court allows you to change it, the state does not allow it on the one document that it’s most required everywhere you go, so for the rest of our life we have to carry a copy of the legal court order with the birth certificate and explained the whole name issue where ever you go and is required. If anyone finds a way to do otherwise, please do share because I have exhausted all possibility to do it and still haven't find a way. :(

Whose name goes on the birth certificate?

i am afraid that when my baby is born my name will not be on the certificate! my girlfriend is trying to divorce her husband but he will not sign and is trying to get shipped to iraq just so she can't divorce him, and she is prego with my baby! my question is since she might still be married will my name be able to go on the certificate or will his name have to be on there cause they are still married!?

I saw the line and decide i will not go inside and thought came in mind what i will reply to my wife then.Step-8 I stand in line more than three hr and when my turns come .. luckily lunch time is started.Step 9 - Finally my turns come and uncle on a window. My Name Is Spelled Wrong On My Birth Certificate?

JM, I wasn't called Jamie or Jamie Lynn by everyone. My parents and closer family that I lived with all called me Jamia. For example, just because your name is Nicollette and you go by Nicole in your childhood, doesn't mean you would want to be known as Nicole for your entire life. Billions of people (ie Margaret to Maggie, Joanne to Jo, Alexandra to Alex) have a nickname in their childhood and personal lives but go by their full name for their professional and adult lives. Only people like teachers and classmates and such so who didn't know any better called me Jamie or Jamie Lynn. And if my mom had intended to name me Jamie Lynn, she wouldn't have called me Jamia or told me that my name was spelled wrong on my birth certificate.

You just have to fill the form for change in a birth certificate.For that approach your municipality or any other places as the case may be.,where the work of this is done.You can go online also for the same.if service is provided.Application must supported with documents.

Is it a crime to put a man's name on a birth certificate when he is not the child's father, and he did not consent for his name to be put on the birth certificate?

Yes, but highly unlikely to be prosecuted unless she's silly enough to tell people she committed fraud.Back in the 80s the country I was living in was putting pressure on unwed mothers to name a father so that they could claim child support from them instead of relying on the state.  The mother would sometimes name the richest guy in the village as the father so she'd get a better amount of child support.It would then be up to the guy to spend $1000 on a paternity test to prove that he wasn't the father.  In the meantime, he'd still be paying child support, and child support isn't refundable under any circumstances (until that point, he is legally the father, with all that entails.)This is one of the legal loopholes where you can be made out of pocket  by a significant amount for no fault of your own and no recourse to get it back.

The first name of my mother is wrong on my birth certificate. What can I do to correct that?

For correcting details in birth certificate you require correction letter from hospital (if event occurred in hospital) , one identity proof of individual in whose name correction required and an application to the registrar stating the above details and correction sought along with the copy of birth certificate, you can submit these documents in the office of concerned registrar. He will issue the corrected certificate.
