It doesn't matter. I don't think there is a standard. Just use small and common words/names. You don't want to use a word/name that could be mistaken for another letter. You wouldn't want to say "C as in Cathey" since it could also be spelled Kathey. Instead use "C as in Cat".

Yes.Apple, in my opinion, really hit peak autocorrect in iOS 9. Every version of iOS and macOS since then has had autocorrect that is progressively more aggressive, and makes things worse. Autocorrect in Mojave and iOS 12 will actually change correctly spelled words.It will also do this thing—this incredibly annoying, stupid, irritating, misguided, brain-damaged thing—where if it sees you capitalize a word a certain number of times, it will always capitalize that word even if you don’t want it to.So if you talk about the movie The Color Purple enough times, autocorrect will automatically start capitalizing the word “purple” every time you use it. The most casual moment of introspection on the part of the developers should have revealed that this is a terrible, terrible idea, yet here we are.I always turn off autocorrect in macOS, because it’s evil and needs to die in a fire.Somewhere in Cupertino, developers working on autocorrect have to justify their combined salary, so they keep making progress in the wrong direction.
