I'm pretty sure this mostly occurs during affiliate breaks. Basically, some commercials are played out by a network, where as some are played out closer to you - by the local affiliate, by the cable or sat company etc.  When a network enters a commercial break, it will play a commercial or 2 of its own, and then 5 sec before the next one there is a tone sent that signals that the downstream affiliates can over ride. This is how you get ads for local car dealers during nfl games. Ads are bought at both the network and affiliate level, and are rarely if ever bought at specific times, or coordinated between the affiliate and network level. If a given ad is being pushed hard, it makes sense that sometimes a network runs it, and then your local affiliate runs it too. So, Comcast viewers in la might see the same ads back to back during the us open on espn, but not Comcast viewers in the bay area, or directv viewers.

The agreement between stations and cable operators have created a 2-tier advertising structure. The first tier are guaranteed national distribution commercial breaks where any spot put there shows nationally.The second tier are local insert optional distribution. In other words, in those breaks the cable operator (or satellite system) is allowed to insert advertising that it has sold for local distribution. The network will already have advertising in that segment - but the local is allowed to cover it over. If you buy this second tier on a cable network, it costs less & you are accepting the random possibility your advertisement will not been seen in x% of the markets.You may notice that many of the spots with the glitch you mention are for local advertisers - car glass replacement, auto dealers, etc. Many are also direct response television ads (although we've never found local inserts on cable to be cost effective).There is a similar arrangement on the part of national broadcast networks with the local stations.So, that brief flash happens when there's a local insert - and where there's error. It's not CNN's fault - it's the local cable provider or satellite system.

There isn't actually a set number of breaks for a hour or half hour show on American TV. Half hour shows often have an open, A and B block, then a close. But that is not necessarily standard for all shows. I have mainly worked with syndicated programs, and those breaks are further broken down into national and local breaks. You have probably seen a break, followed by a short show bump, then another break. That is a common commercial configuration for syndi shows, but hardly the only one. Honestly, the show distribution company handles this; they are the ones taking the programs and editing them for air. Network airings of Family Guy, say, will not be formatted the same way when they air on a local station in reruns.I also help manage a community channel, and plenty of the programs we get are self- contained, and we only do ad insertion at the end of the show to hit the top or bottom of the hour.
