I am assuming that you are talking about installing Android Lollipop using the Official Factory images. If that is the case, the answers for your questions are No and No :-)You might need to have an unlocked boot-loader to install the official images, but as it is a Nexus, this can be easily locked. So you do not have to worry about warranty problems.

Why does my phone say it's rooted but I never rooted my phone?

I have no clear answer for that, this is mere a suspicion from my part:It's your device bootloader unblocked?Why I do state this: I just tried installing a banking app in my Nexus 7 2013 tablet and, while yes it has been rooted before and has had many versions of Android, at the moment it has installed a 100℅ stock firmware, Android 6.0.1 downloaded from Google and flashed entirely by me. So, it is definitely not rooted now, yet this app still says it doesn't run in rooted devices as this one.The only thing left that could be being validated somehow by this app is if the phones is bootloader unlocked which it is....Opinions?
