It brings you as close to death as a person can get (without actually dying). You’re paralyzed (after you’re sedated) and then there’s the tricky next 10 seconds while the anesthesiologist, with the assistance of a nurse giving cricoid pressure, has to wrangle a tube into your airway. This causes pause of everyone else in the OR. People are aware that EVERYTHING COULD GO WRONG at that moment.While the time is seconds, and it’s minutes needed to incur brain damage, it feels like a bit of a close shave when it’s your brain on the line.Sometimes the tube doesn’t go in smoothly, and it’s the minority of anesthesiologists (at least that I’ve worked with) that can be calm in this moment. The tense silence is usually broken by the doctor accusing the nurse of doing the cricoid pressure incorrectly, while there’s a general shuffling of tubes and tongue blades.As the nurse in this situation it’s a bit disheartening too watch one of the most highly trained people in the OR spaz out. You can read their expression. You know they’re thinking, “how much longer can I fiddle with this airway before I need to bag them?” And it feels like those seconds stretch out into hours with everyone casting an anxious eye at the pulse ox.They always get it, at least in my experience. But there’s the risk of things going South. Joan Rivers was a tragic example of that.In my last surgery, something did go South. I woke up to a heart rate in the upper 180’s while violently shaking.
