When I was around 13–17 I would swear a lot in school. I do not know why, but it definitely wasn’t for attention. I am good at English but when it comes to speaking, I just freak out, mix up the words and forget the content. What can I do to improve it?

I totally understand what you mean as it happens to me too (all the time!). The only way to remedy this, is to get out of your comfort zone and to be on the spot as much as you can. Facing your fears is the only way out. You can ask your friends to converse with you on a random topic or you might just interview them for fun. Once you get fluent and comfortable with your friends, try making small talks with colleagues or some acquaintances. The more you converse, the more you become confident. Also standing infront of the mirror and talking to yourself (as crazy and silly as it sounds) actually helps you. Moreover by doing so, you can work on your body language simultaneously.
