If it never happened, how would life in America be like today?

You don't have to answer all of the questions, just the ones you want.

Where did the concept of not shooting medics come from? Why would any army, from a pragmatic point of view, follow this policy? Is it entirely enforced by retaliation, e.g. "don't shoot our medics or we'll shoot yours, and you like having medics"?

The purpose of war is not to kill the enemy. The purpose of war is to prevent the enemy from being able to continue to fight. Killing the enemy's soldiers is one way to do that. It's not the only way. There are many other strategies that warring nations also use: Destroy the enemy's infrastructure. Destroy the enemy's equipment. Cripple the enemy's communication. Prevent the enemy from getting supplies and raw materials. Destroy the enemy financially. You can't just look at war as "kill the other guy." There's a lot more to it than that. When you kill an enemy soldier, you remove one person from fighting. When you wound an enemy soldier, you remove more than one person from fighting. The medics who rescue him, the doctors and nurses who treat him, all are people who are not fighting. Every dollar spent treating him is a dollar not spent on building tanks and guns. Every logistics capacity that is transporting medicine and bandages is not transporting tanks and guns. From a purely return-on-investment perspective, a severely wounded enemy is better for you than a dead enemy.By allowing medics to rescue the enemy's wounded, you are putting a strain on the enemy's resources.

How can people want world peace, yet carry such hate for other groups?

Wanting peace means changing at least two things. The first is changing how the enemy sees your tribe. The second thing is changing how you see the enemy.Expand that idea to societies, and it’s obviously next to impossible. However, it’s pretty interesting that there is generally peace among most tribes and peoples today, even between those that hate each other. Economists say that the reason for that is trade, but it also goes beyond that.Being seen as a peaceful society is not just good for business. It’s good for the society’s quality of life, ability to make progress, and sets up a positive legacy.
